What are CBS and IS?

Community-Based Supports (CBS) and Individual Supports (IS) provide personalized assistance for adults with developmental disabilities at home and in the community. Support may include everyday routines, meal preparation, shopping, social activities, and building independence. Your support coordinator can help determine which service fits your needs and service plan.

Which areas do you serve?

Hearten offers CBS and IS services throughout New Jersey. Staffing availability varies by location and schedule. Contact us to discuss support in your area.

Can a parent or family member provide support?

Yes—eligible parents and family members may become paid support professionals through Hearten. We guide you through hiring and required training and handle payroll, billing, and ongoing agency support. Contact us to explore whether family hire is an option for you.

How are services funded?

Services are funded through New Jersey’s Division of Developmental Disabilities (DDD) Medicaid waiver programs for eligible individuals. Your support coordinator helps arrange services within your approved budget and Individualized Service Plan (ISP). We’ll work with you and your support coordinator to help you get started.

Can you support someone with behavioral needs?

Yes. Hearten supports individuals with behavioral needs, with guidance from a Board Certified Behavior Analyst (BCBA). We work with the individual, family, and support team to understand their needs and determine how we can provide appropriate support at home and in the community.
