Community-Based Supports
Hearten offers personalized in-home support through the DDD Community-Based Supports (CBS) and Individual Supports (IS) programs. Each participant is matched with a Direct Support Professional (DSP) who provides tailored one-on-one assistance. This individualized approach allows for engaging, person-centered activities that cater to the unique needs and interests of individual, both at home and within their local community.

What is Community Based Support?
Community-Based Support (CBS) in New Jersey is a service provided by the Division of Developmental Disabilities (DDD) to help individuals with developmental disabilities become more independent and actively engaged in their communities. This service promotes life skills, social interaction, and participation in community-based activities.​

How to Access Community-Based Support in NJ:
To access CBS services, the individual must:​

Be enrolled in one of the DDD Medicaid waiver programs:

Supports Program (SP) – for individuals living with family or independently.

Community Care Program (CCP) – for individuals requiring more intensive support, including residential services.

Have an Individualized Service Plan (ISP) that outlines the goals and the types of CBS services needed.
